EFFECT OF HEALTH EDUCATION ON KNOWLEDGE ATTITUDE AND MALARIA TREATMENT – SEEKING BEHAVIOUR OF CAREGIVERS OF UNDER FIVE CHILDREN IN RIVERINE COMMUNITIES IN LAGOS STATE

Abstract

Background: Malaria accounts for one million deaths among children under 5 annually in Nigeria and in most sub-Saharan Africa despite decades of efforts to control it. It has been shown that improving home treatment and preventing delays in seeking treatment by educating caregivers to respond promptly and appropriately when their children have fever can decrease malaria related morbidity and mortality. This study looks at the effect of Health Education on Knowledge, Attitude, prevention and Home Management of Malaria (HMM) among caregivers of children under 5 years in Ibeshe riverine village in Amuwo Odofin LGA, Lagos and compares the result with a control group, in Iyagbe riverine village in Ojo LGA.
Materials and Methods: The study was a community based intervention study. One riverine village each from the riverine villages in Amuwo- Odofin and Ojo LGA was selected by balloting into intervention and control groups. A total of one hundred and seventy one caregivers with children aged 6 months to 5 years of age from households were selected from each of the intervention and the control group through a multistage sampling technique. Baseline information was collected through an interviewer administered structured questionnaire, This was followed by intervention, in form of health education session in the intervention community with data collected in the intervention community only. Post intervention data were collected after 3 months from 165 and 166 caregivers from intervention and control groups respectively. (Attrition rate was about 3%). No health education was given to control group until after the study was completed.

Results: Socio-demographic indices shows that the caregivers in both intervention and the control groups were characterized by low educational levels, low income level. 46.8% and 36.8% of the caregivers children in the intervention and the control group respectively suffered malaria (fever serves as proxy for malaria) at pre-intervention and 18.8% of children at post-intervention.
The mean knowledge scores at pre-intervention were 9.21 ± 3.75 and 9.29 ± 3.73 for intervention and control group respectively. There was no statistically significant difference in the knowledge score in both groups at baseline. However at the immediate post-intervention, the mean knowledge score increased significantly from 9.29 ± 3.75 to 14.45 ± 2.37 in the intervention community. Comparison of the mean score at pre-intervention, immediate and late post intervention was statistically significant P<0.001. The mean attitude score also increased at post intervention from the pre intervention score of 3.19 to 4.81 at 3 months post intervention. The attitude score of the control group at post-intervention increased but less than that of the intervention group.
At post intervention , health education had an impact on knowledge of malaria prevention and home management of malaria (P<0.001, and P<0.001,respectively) more than the control. The intervention could not appreciably reduce the practice of herbal administration by caregivers to their children with malaria as first line treatment option but was able to reduce appreciably the use of 7 up and salt mixture, Septrin /Amoxycillin (antibiotics) for the management of malaria in the intervention group compared with the control group.

Conclusion: Health Education impacted positively on caregivers knowledge and attitude on malaria treatment-seeking behavior. Caregivers should be involved in malaria control activities at the community level.
