Abstract

The observance of universal precautions against HIV/AIDS and other blood-borne pathogens is of absolute necessity to the healthcare worker who is often in contact with blood and body fluids of patients. This study was undertaken to determine the effect of health education on universal precautions against all blood borne or body fluid pathogens on health services delivery practices of healthcare workers in a Nigerian teaching hospital.
Methodology: An intervention study on 327 healthcare workers, conducted at NAUTH, Nnewi and UNTH, Enugu, in South-Eastern Nigeria, using close-ended, structured questionnaire. The healthcare workers were selected by stratified random sampling technique. Health education on universal precautions was given to the intervention group (NAUTH), while the control group (UNTH) received a non-HIV/AIDS-related health education lecture. Data were entered into the computer using EPI-INFO version 6 and data analyzed using the same software.
Results: Pre-intervention knowledge, attitude and practice mean percentage scores were not significantly different between the study group and the control group (75.8% vs 75.4%; 50.2% vs 44.1%; 48.8% vs 43.4% respectively). P>0.05. However, immediate post-intervention mean
percentage scores for knowledge and attitude were significantly higher in the study group compared with the control group (97.5% vs 73.8%, and 55.4% vs 42.0% respectively). P<0.05. Similarly, mean percentage scores for practice was significantly higher at 3-months post-intervention for the study group (73.5%) compared to the control group (46.3%). P<0.05, using the z-test statistics.
Conclusions and Recommendations: The poor knowledge, attitude, and practice of universal precautions amongst healthcare workers shown in this study underscores the need for continuous health education of healthcare workers on the subject, and the formulation and implementation of policy on universal precautions against HIV/AIDS in health facilities. A structured, needs-assessed education; monitoring; improved availability of resources and disciplinary measures towards the observance of universal precautions against HIV/AIDS are therefore recommended for every healthcare institution.
