ASSESSMENT OF LIFESTYLE MODIFICATION MEASURES AND THEIR EFFECT ON GLYCEMIC CONTROL IN ADULT TYPE 2 DIABETES PATIENTS  IN DALHATU ARAF SPECIALIST HOSPITAL LAFIA

SUMMARY

Background:

Each year, 3.2 million people around the world die from complications associated with diabetes.19 The prevalence of diabetes for all age groups worldwide was estimated to be 2.5% in 2000.7 In Nigeria, the present prevalence of diabetes was reported by International Diabetes Federation to be 3.9%.11 More than 1.2 Million Nigerians live with diabetes.15 Adoption of a healthy lifestyle may provide a protective effect against diseases such as diabetes mellitus. Apart from that, lifestyle interventions bring about improved quality of life for diabetes patients.

Objectives: The general objective of the present study was to assess the effect of lifestyle modification measures in type 2 diabetes patients in Dalhatu Araf Specialist Hospital, Lafia (DASH)  in order to recommend management guidelines for these patients.

Methodology: This study was a randomized controlled trial. A total of 84 participants were recruited and randomized into two groups. The control group (44 participants) was on hypoglycemics alone, while the intervention group (40 participants) was on hypoglycemics and lifestyle modification measures. Both groups were followed up for 12 weeks and data was collected on their perception and practice of lifestyle measures, BMI and blood glucose levels.

The data of all the study participants were analysed on intention-to treat basis.

Results: Thirty (68.2%) participants in the control group against 25 (62.5%) in the intervention group perceived lifestyle modification measures as helpful. Thirty-three (75.0%) of the study participants in the control group as against 11(27.5%) participants in the intervention group reported that they did not practice any of the lifestyle modification measures.

The mean fasting plasma glucose in the control group reduced from 11.2mmol/L to 8.6mmol/L while for the intervention group it reduced from 11.3mmol/L to 6.1mmol/L.

There were reductions in BMI in the control and intervention groups. However, the reductions were more appreciable in the intervention group. Comparison of the mean fasting plasma glucose in the control and intervention groups showed a significance of 3%, p value= 0.03. Barriers to the practice of lifestyle measures included factors such as lack of time, financial constraints, body pains, and lack of a partner to exercise with.

Conclusion: Lifestyle modification measures are effective in the management of type 2 diabetes.

Screening for diabetes should be an integral part of the baseline assessment of adult Nigerians attending GOPD in all hospitals to facilitate early detection and appropriate preventive measures. These interventional measures should be considered by health planners and policy makers in their formulation of policies and designing of programs that focus on reducing the burden of diabetes in Nigeria.

In the process of treating diabetes patients, the family should be involved to support the counseling plan because, family support enhances recovery from illness.
