A COMPARATIVE STUDY OF CLIENTS’ PERCEPTION OF THE QUALITY OF FAMILY PLANNING SERVICES AMONG WOMEN ATTENDING URBAN AND RURAL HEALTH FACILITIES IN LAGOS STATE, NIGERIA

Abstract

Quality of care, a client-centered approach to providing high-quality health care as a basic human right, has emerged as a critical element of family planning and reproductive health programs. Empirical evidence has shown that quality of family planning services affect clients’ usage of methods effectively, clients’ continuous use and whether people start family planning at all.
The objective of this study was to identify and compare clients’ perception of the quality of family planning services in urban and rural health care facilities in Lagos State. A crossectional descriptive study was carried out from April to June 2005 in Surulere (urban) and Ikorodu (rural) local government areas. Data collection was by exit interview of clients after consultations or counseling sessions at family planning service delivery points in a private area out of ear shot of clinic personnel.
Two hundred and forty respondents, 120 from each local government area were interviewed. The mean age of respondents was 34.3years and 33.6 years in Surulere and Ikorodu local government areas respectively. All respondents (100%) in Surulere local government area were currently married and all had some form of education compared to 96 percent and 93.6 percent of those that were currently married and had some form of education respectively in Ikorodu local government area. More than three quarter of respondents, 84.2 percent and 83.3 percent of respondents in Surulere and Ikorodu local government areas respectively were currently employed. The mean number of living children reported by respondents was 3.1 and 3.4 in Surulere and Ikorodu local government areas respectively.

Respondents’ perception of the service provider showed that 94.2 percent of respondents in Surulere local government area reported that they were treated very well by the provider compared to 60 percent of respondents in Ikorodu local government area.
The mean waiting time reported was 25.8 minutes and 32.4 minutes in Surulere and Ikorodu local government areas respectively. All the respondents (100%) in Surulere local government area had enough privacy compared to 91.7 percent of respondents in Ikorodu local government area. There was similarity in the cost paid for services in the two local government areas, cost ranges between N 50 and N800 in Surulere local government area while in Ikorodu local government area it ranges between N50 and N550.

Providers in Surulere local government area performed better in the areas of clients’ perception of interpersonal relations than those in Ikorodu local government area because all respondents (100%) from Surulere local government area reported they were treated with dignity and respect compared to 97.5 percent of respondents in Ikorodu local government area. Also 92.6 percent of respondents in Surulere local government area were given appointment for follow up compared to 78.3 percent of respondents in Ikorodu local government area.
Training and retraining programmes for providers on the technical competence, interpersonal relations and counseling and communication skills should be organized for FP providers in all the health facilities offering FP services in both urban and rural local government areas of Lagos State. In addition, Lagos State government should redeploy / employ more staff to the rural health facilities and provide basic amenities to the health facilities.
