PREVALENCE OF ASYMPTOMATIC BACTERIURIA AMONG THE ANTENATAL POPULATION AT THE JOS UNIVERSITY TEACHING HOSPITAL (JUTH), JOS

Abstract

BACKGROUND: Asymptomatic significant bacteriuria in pregnancy is
a major obstetric risk factor, if left untreated. It is associated with
poor maternal and perinatal outcomes. Evidence from research has
confirmed the cost-effectiveness of early screening and treatment of
asymptomatic significant bacteriuria in pregnancy.
OBJECTIVE: To determine the prevalence, bacteriology, antibiotic
sensitivity of isolates and predictors of asymptomatic significant
bacteriuria among pregnant women attending the antenatal clinic at
JUTH.
METHODOLOGY: This was a clinic-based cross-sectional study
involving 250 eligible pregnant women attending the antenatal clinic
at JUTH. The women were selected using a systematic sampling
technique. Data were collected from cultures of two clean-catch urine
specimens and a third culture to ascertain clearance of asymptomatic
bacteriuria after antibiotic treatment. Data were analyzed using Epi
info 2007 (version 3.4.1) statistical software. Tests of association
between variables were performed using chi-square and t-test for
categorical and continuous variables respectively. A p-value of 0.05
was taken as significant.

RESULTS: Twenty-six (26) of the 250 pregnant women had
asymptomatic significant bacteriuria, giving a prevalence of 10.4%.
The predominant isolate was Escherichia coli, accounting for 38.5%
of all isolates. All isolates were highly sensitive to levofloxacin,
ciprofloxacin, gentamycin, streptomycin, ceftriaxone, cefuroxime and
most sensitive to ceporex. All were resistant to cotrimoxazole.
Ceporex rendered all women with significant bacteriuria bacteria-free
after treatment. The main predictors of asymptomatic bacteriuria
were low parity (≤2) and increasing gestational age. All pregnant
women treated with ceporex remained bacteria-free throughout the
follow-up period.
CONCLUSION: Given the high prevalence of asymptomatic
significant bacteriuria in pregnancy and the high sensitivity of isolates
to available antibiotics in our setting, early screening at booking could
help in initiating early therapy in order to avert possible adverse
pregnancy outcomes.
