ANTIOXIDANT STATUS AND CERVICAL CANCER: A CASE-CONTROL STUDY AT LAGOS UNIVERSITY TEACHING HOSPITAL.

Abstract

Carcinoma of the cervix is the most common gynaecological cancer and the leading cause of female genital cancer mortality in Lagos University Teaching Hospital. Some studies have implicated oxidative stress in the poor quality of life and progression of cervical cancer.
Aim: To investigate the role of oxidative markers in the severity of cervical cancer at Lagos University Teaching Hospital.
Study design: This was a case-control study.
Methods: Antioxidant levels (glutathione for endogenous antioxidant status and vitamin C for exogenous antioxidant status) and malondialdehyde (MDA), a marker of lipid peroxidation were measured in 80 patients with cervical cancer and 120 healthy controls. The cases were divided into 4 different groups according to their stages (I, II, III & IV). Ten milliliters of venous blood was collected from the cervical cancer patients before treatment, and also from the healthy controls.
Results: This study found marked elevation in MDA (p <0.001) levels among the cervical cancer patients compared with controls. The levels of serum MDA among the cases increased significantly as disease stage progressed (p < 0.001). There was also marked reduction in the levels of erythrocyte GSH (p < 0.001) and serum vitamin C (p < 0.001) among the cervical cancer patients. Also, a statistically significant (P < 0.05) negative correlation (r = – 0.67 and  r = – 0.46) was found between GSH vs MDA and vitamin C vs MDA respectively among the cervical cancer patients compared with controls (r = – 0.337 and r = – 0.17 respectively).
Conclusions: This study has shown that the patients with cervical cancer have low levels of antioxidants (reduced GSH and vitamin C) and high lipid peroxidation (MDA), which is a marker of oxidative stress; and this worsened as the disease stage progressed. There is a need for antioxidant supplementation in these patients to reduce oxidative stress
