A COMPARATIVE STUDY OF THE QUALITY OF LIFE OF FAMILY CAREGIVERS OF CHILDREN WITH AND THOSE WITHOUT OROFACIL CLEFTS ATTENDING TERTIARY HEALTH INSTITUTIONS IN LAGOS STATE

Abstract

Introduction: The caregiver role in families with a disabled member can dramatically impact on the individual, other members and the whole family. This study compared the quality of life of family caregivers of children with and those without orofacial clefts attending tertiary hospitals in Lagos state.
Methodology: A mixed method, comparative, cross-sectional study in which family caregivers of OFC children were consecutively recruited from cleft clinics and non-OFC children were selected by systematic sampling from the immunization clinics of tertiary institutions over a 3-month period. The interviewer administered questionnaire used consisted of the IOFS and family APGAR. Three focus group discussions were also conducted. Quantitative data was analyzed using SPSS version 17 and FGD by framework analysis.
Results: Caregivers’ mean age was 31 years in both groups. Children of non-OFC caregivers were more exclusively breastfed (61%Vs 21.5%)(p<0.01) with a better QOL (39% Vs 9.3% p<0.01). The highest predictor of QOL was the social dimension (Beta= 0.674, p<0.01) with related factors as increased duration of breastfeeding (r=-0.190 p<0.01) and later diagnosis of cleft (r=-0.203 p<0.05). Predictors of good QOL were exclusive breastfeeding (Beta=-6.383, p<0.001), child’s age (Beta=0.136, p=0.013), income (OR=71.566, p=0.043) and professional support (OR=105.903, p=0.048). Both caregiver groups had normo-functional families and functionality was better in Non-OFC caregivers 8.07±2.32Vs 7.04±2.53) (p<0.01).Family functioning was a predictor of good QOL of family caregivers (Beta=-1.253, p<0.001).
Conclusion & Recommendations: The quality of life of family caregivers of non-OFC children was found to be better than for OFC family caregivers. Recommendations were for establishment of more social support groups, increased community awareness to reduce stigmatization of the birth defect and continued promotion of exclusive breastfeeding 8.07±2.32Vs 7.04±2.53) (p<0.01).Family functioning was a predictor of good QOL of family caregivers (Beta=-1.253, p<0.001).
