THE VALUE OF ONE AND TWO DOSE REGIMEN OF CEFTRIAXONE AS PROPHYLAXIS IN CAESAREAN SECTION AT FEDERAL MEDICAL CENTRE, ABEOKUTA
Abstract
Objective: To compare the efficacy of a single-dose regimen with twodose regimen of intravenous ceftriaxone as antibiotic prophylaxis for caesarean section.
Design: Randomized controlled trial.
Setting: Department of Obstetrics and Gynaecology, Federal Medical Centre, Abeokuta.
Population: Pregnant women undergoing caesarean section that met the study inclusion criteria.
Method: One hundred and four women who consented to the study were randomly assigned to receive intravenous ceftriaxone either as a single-dose regimen or as a two-dose regimen for antibiotic prophylaxis at caesarean section. Maternal bio-data and infectious morbidity outcome were compared between the two groups.
Main outcome measure: Incidence of post-caesarean section morbidity, postpartum fever, postpartum endometritis, wound infection and urinary tract infection.
Results: The incidence of infection related morbidity following caesarean section at FMCA was 12.5%; 6.8% in the double dose group and 5.8% in the single dose group. Postpartum fever occurred in 4.8% and 5.8% of single dose and double dose respectively, while wound infection was noticed in 2.9% and 2.0% of single dose and double dose respectively. Also, the incidence of postpartum endometritis was 1.9% and 2.9% in the single dose and double dose respectively, while 1% of the single dose group only had UTI.
Conclusion: No statistically significant difference between the single dose and double dose regimen {P-value of 1.000 and odds ratio of 0.839}, hence the single dose is as effective as the double dose.
